PATIENT INFORMATION

Patient information is obtained through a telephone interview; details about the pain complaint, prior treatments and the impact of the pain on the individual’s life and functioning are obtained. Insurance information is also obtained at this time. The Pain Management Center participates with most insurance providers. Exceptions can be discussed during the telephone interview. Patients are asked to contact previous care providers for records relating to the pain complaint, including reports of diagnostic tests (x-rays, MRIs, CT scans, etc.), operative reports, office notes detailing previous treatments, and records from other pain management centers, if applicable.

REFERRALS

Several insurance carriers will require a referral from the primary care physician or doctor responsible for directing the patient’s care. Should assistance be required in obtaining this referral we will be happy to help.


INSURANCE COVERAGE

Most insurance carriers will cover pain management services, provided appropriate referral and prior approval have been arranged for elective consultations and procedures. We participate with many health care plans and will assist you to ensure adequate coverage.
